Support & Sign-Posting

Support

The project offers support to all clients; support is initially delivered by either Stuart or Kathryn. Support takes many forms from the simplest through to a care planned approach. It can range from emotional support for past and present issues to advocacy to housing providers or benefits agency. Support levels can be very intensive 1-1 sessions or on a ad-hoc basis as and when an issue impacts on a clients life.

It is usual for new comers to the project to require a greater level of support usually due to the unmanageability of their lives compared to those who have had time to stabilise and address their issues. The support that offered is tailored to each individuals needs and incorporated in a care plan. This is a live document, which can be adaptable and easily changed according to client need. Clients often dip into the service for support during times of personal stress or upset.

The project will refer to other agencies to obtain this extra support i.e. Action Housing provide Tenancy support and offer a specialised support service which helps clients sustain accommodation, Dual diagnosis team (On Track) assist clients who may be experiencing mental health issues alongside substance misuse.
